I understand that transportation to and from classes and special events is the responsibility of the parents and students. Checking this box is mandatory and acknowledges you understand the Transportation Policy.

By checking this box, you acknowledge that you and your child understand that more than three unexcused absences may cause your child to be dropped from the JAM program. Please get in touch with JAM by email if your child is expected to be absent or late to ANY scheduled JAM class or event.

By checking this box, you acknowledge that you will notify the JAM staff if the student has any health conditions or allergies, including food allergies, that the program staff needs to be mindful of. There is a possibility that snacks may be served at JAM classes and events, so students with special dietary needs are encouraged to bring their snacks.

JAM does not retain a qualified health professional on staff. The JAM staff and JAM volunteers will not be administering any medications to any student at any time. Emergency Contact information will be collected from parents at the mandatory orientation.

By checking this box, I acknowledge and agree that I am solely responsible for my child’s safe and responsible participation in all JAM activities.

I agree for myself/​my child and on behalf of my/​my child’s heirs, representatives, successors and assigns (“user’s parties”) that JAM, including its owners, partners, members, directors, officers, employees and agents (JAM Parties) will not be liable for any damages or injuries user parties may suffer in or around JAM or while participating in JAM sponsored activities. I agree on my/​my child’s behalf that my child nor I will make a claim against, sue or attach the property of a JAM party or affiliate thereof. I agree that JAM will not be liable for any loss, theft, or damage to my/​my child’s property in or about JAM's Activities.

By checking this box, I acknowledge and agree that:

I, the parent or legal guardian, grant permission for Tellico Plains JAM, its staff, volunteers, sponsors, and affiliated media organizations to photograph, videotape, and record my child during Tellico Plains JAM activities and events.

I understand that these photographs, videos, recordings, and related materials may be used for educational, promotional, publicity, fundraising, and informational purposes. This includes, but is not limited to:

• Social media platforms
• Websites and online publications
• Newspapers and magazines
• Television broadcasts
• Radio broadcasts
• Printed promotional materials

I understand that my child's name may be used in connection with these materials.

I release and hold harmless Tellico Plains JAM, its officers, directors, employees, volunteers, sponsors, and participating media organizations from any claims arising from the use of such photographs, videos, recordings, or publications.
